This form is designed for business owners/managers to submit their business and perk information to be included in the Pirate Perks program.  If you are already in the program and need to renew or update you information, go to Renew/Update Pirate Perk.

Please note, it is preferable for business to be within Pitt County, NC or the surrounding counties.  You will receive a response confirming of denying your perk application within 4 weeks.
